What It Is

What is testosterone replacement therapy and who is it for?

Best Age to Start Testosterone Therapy

Testosterone is the primary male sex hormone responsible for energy, muscle mass, bone density, mood, cognitive function, and sex drive. After age 30, testosterone levels naturally decline at roughly 1% per year. For many men this gradual drop goes unnoticed for years, or gets blamed on stress, aging, or overwork.

Low T, also called hypogonadism, is clinically defined by a blood testosterone level below a threshold your provider determines alongside your symptoms. Labs confirm the diagnosis, symptoms alone don’t. Every TRT program at PUUR starts with a panel covering testosterone, estrogen, and PSA before any prescription is written. The goal isn’t to maximize testosterone but to bring it into a healthy, optimized range where your body functions the way it should.

How It Works

How the program works.

1

Book Your Consultation

Schedule a consultation with your provider. We review your symptoms, health history, and current medications. If you have recent qualifying labs from your PCP, bring them and the $150 lab fee is waived.

2

Complete Your Lab Work

If labs are needed, your provider orders your panel at the consultation. We require testosterone, estrogen, and PSA before any prescription is written. Labs are typically returned within a few days.

3

Choose Your Program and Start

Once your labs are reviewed and your prescription is written, you choose your program. Pharmacy pickup patients receive injection training. In-clinic patients schedule their first weekly appointment.

4

Ongoing Monitoring

Your provider monitors your labs and symptoms and adjusts your dose as needed. Most patients notice meaningful improvements in energy, mood, and strength within the first 4 to 8 weeks.

Consider Adding HCG

Protect fertility and testicular function with HCG.

TRT suppresses the body’s natural LH signal, which causes the testes to stop producing testosterone on their own over time. Many men on TRT add HCG to their protocol to maintain testicular size and function and preserve fertility. The same $150 lab panel covers both programs, no extra labs needed if you qualify for both.

How do I know if my testosterone is actually low?

The only reliable way to confirm low testosterone is a blood test. Symptoms alone overlap too heavily with other conditions to use as a sole diagnosis. That is why we start every program with a lab panel. If your levels come back in the low-normal or below-normal range and you are experiencing symptoms, your provider will discuss whether TRT is appropriate for your specific situation.

Yes. Testosterone replacement therapy suppresses the body’s natural signal to produce testosterone, which also suppresses sperm production. For men who are actively trying to conceive, TRT is not recommended without careful consideration and guidance. If preserving fertility is a priority, discuss this with your provider at your consultation. Adding HCG to your protocol is one approach that can address low T while better preserving fertility.

When properly managed with regular lab monitoring TRT is considered safe for most men. The key word is monitored. Your provider tracks your red blood cell count, estrogen levels, and PSA on an ongoing basis to catch any changes early and adjust your protocol accordingly. Unsupervised or improperly dosed TRT carries real risks. Medically supervised therapy with regular labs is how those risks are managed responsibly.

What is the difference between the two programs?

The pharmacy pickup program gives you the flexibility to self-inject at home on your schedule. Your monthly fee covers your provider consultations, follow-up labs, and prescription management. The medication itself is purchased separately at your pharmacy. The in-clinic program brings you in weekly for your injection, which is administered by your clinician. Both programs include provider oversight and follow-up labs. The right choice depends on your preference for convenience versus clinical support at every step.

Most patients notice improved energy and mood within the first 3 to 4 weeks. Strength and body composition improvements typically follow between weeks 6 and 12 as your levels stabilize. Sexual function improvements often appear within the first month. Full optimization at your correct dose can take 3 to 6 months as your provider fine-tunes your protocol based on ongoing lab results.

Yes. If you start with the pharmacy pickup program and decide you prefer the structure of weekly in-clinic visits, or vice versa, discuss it with your provider at your next check-in. Switching is straightforward and your provider will manage the transition.
